Dùng cho kết quả tự đánh giá tiêu chuẩn chất lượng cơ bản của bệnh viện theo TT 35/2024/TT-BYT — mỗi năm một lần, thực hiện trong Quý I của năm liền kề tiếp theo (Điều 1 khoản 3 điểm a, b), và cho báo cáo chỉ tiêu tiêm chủng, sàng lọc.
